NJROTC Forms: Sheet
|
New Students
Welcome to Admiral Farragut Academy! To facilitate your check-in, the following is a short brief about the Academy and the NJROTC program. Our goal is to help ensure a smooth transition and provide the necessary information to properly place the new cadet into the Academy's battalion structure.
In accordance with Academy policy, all Upper Division cadets are automatically enrolled into the NJROTC program. As such, Upper Division cadets are responsible for adherence to both the Academy and NJROTC rules and regulations. During the check-in process, you will be asked to complete a few NJROTC required forms. We realize that it appears that you are providing duplicate information- i.e. medical and insurance information to both us and the Infirmary. We apologize for this inconvenience however; we are required under Navy policy to maintain certain information in order to properly administer our NJROTC Unit. The required forms include the NJROTC Standard Release Form (CNET-GEN 5800/4) and the NJROTC Health Risk Screening Questionnaire (CNET Form 1533/106). To expedite your check-in, these forms can be filled out in advance. Please ensure that all questions are completely answered. If a particular question is not applicable to your cadet, please indicate that on the form. The Naval Science Check-in Sheet is also available for download and completion. By completing this form in advance, the check-in process normally goes much smoother and it helps to ensure your individual privacy.
Your cadet will then be entered into the Navy JROTC Unit Management System or (JUMS). Please rest assured that information entered into JUMS is NOT forwarded to the Navy. This information is used to monitor the progress of the cadet and as a central source for emergency information.
The final step in the check-in is the assignment of the cadet to a company. New cadets who play a musical instrument are encouraged to join Band Company. Cadets that have an interest in joining the military, either directly after graduation or following completion of an officer program such as NROTC, are encouraged to join Drill Company. Drill Company normally maintains higher standards for deportment and appearance. Drill Company cadets are normally expected to join one of several drill teams that participate in JROTC field meets, color guard performances, as well as other special events. All other cadets will be assigned to one of three line companies- 1st, 2nd, or 3rd Company. These cadets, as well as all other cadets, will be expected to participate in weekly drill practice(s), school parades, and personnel inspections.
